Employer:
Download, complete and sign the “Employer Authorization Agreement for Pre-authorized Payments“

Employee:
Download, complete and sign the “Employee Direct Deposit Authorization Agreement“

​Fees:

• $3.00 per pay period for up to 5 employees
• $0.50 for each additional direct deposit

For example, 10 direct deposit accounts is only $5.50 more per pay period, in addition to your normal fees.. 

Once you have completed the required forms, you may fax them to your branch office to get started. However, originals must be received in our offices within 5 business days.
