Check Signing Form

Employer:
Download, complete, and sign the “Automatic Check Signing Signature Form” following the instructions on the form for placement of your signature.

Please indicate whether you would like automatic Check Stuffing and Sealing as well.

Then just mail or fax the form to your branch, or scan and email to CPS@cpsgo.com.

Fees:

• Check signing is just $3.00 per pay period* 
• Check stuffing and sealing is $0.25 per check

For example, it’s only $5.50 per pay period to sign, stuff, and seal 10 checks, in addition to your normal fees.
